Insurance Information
We accept most insurances including Medicare and major medical plans. Please call our office and we will be happy to assist with insurance verification.

ANNALEE CHIROPRACTIC APPOINTMENT CANCELLATION/NO SHOW POLICY
Thank you for trusting your health care to Annalee Chiropractic Care. When you schedule an appointment with our office, we set aside enough time to provide you with the highest quality care. Should you need to cancel or reschedule an appointment please contact our office as soon as possible, and no later than 24 hours prior to your scheduled appointment. This gives us time to schedule other patients who may be waiting for an appointment. Please see our Appointment Cancellation/No Show Policy below:
- Effective January 1, 2023, any established patient who fails to show or cancels/reschedules an appointment and has not contacted our office with at least 24-hour notice will be considered a No Show and charged a $45.00 fee.
- Any established patient who fails to show or cancels/reschedules an appointment with no 24-hour notice a second time will be charged a $90.00 fee.
- If a third No Show or cancellation/reschedule with no 24-hour notice should occur the patient may be dismissed from Annalee Chiropractic Care.
- Any new patient who fails to show for their initial visit will not be rescheduled.
- The fee is charged to the patient, not the insurance company, and is due at the time of the patient’s next office visit.
- Please understand that it is your responsibility to remember your appointment dates and times in order to prevent any missed appointments which result in a cancellation fee. As a courtesy, we email reminder for appointments. Not receiving a reminder email from us is not sufficient reason to miss an appointment and the above Policy will remain in effect.
We understand there may be times when an unforeseen emergency occurs, and you may not be able to keep your scheduled appointment. If you should experience extenuating circumstances please contact our Office Manager, who may be able to waive the No Show fee. You may contact Annalee Chiropractic Care during the office hours at the number below. Should it be after regular business hours, you may leave a message. Either phone or email messages left are acceptable.
Late Arrival for your Scheduled Appointment
We understand that delays can happen, however, we must try to keep the other patients and doctors on time. If you are running late, please notify the office.
Any late arrival will shorten your appointment time and will not be made up by running into the next patient’s scheduled appointment. Our staff will call the patient at 5 minutes past the hour of the scheduled appointment to verify the appointment status. If you are 10 or more minutes late for your appointment, the appointment may be cancelled and rescheduled.
